Q: Has anyone encountered a situation where it comes off when using it?

I haven't used it yet. I would suspect you would really need to twist them tight to prevent slipping

What you need is an actual countersink drill bit set. They come with an adjustable hex screw for the bit adjustment. You'll still have the issue of the countersink piece continuing to cut/drill down. Use a piece of masking tape as your gauge/marker as to when to stop. If you're using countersinks then depth shouldn't have to be super accurate anyway if you're using a plug or covering the screw with wood putty. If you're making them flush, use the masking tape and practice a couple of times on a scrap piece.

The interior sleeve spaces the bit from the aluminum assembly. the exposed end of the aluminum may wear over time, but it will always be able to set up to the correct depth.
